Formulation of diagnoses and treatment plan for acute and chronic illness based upon exam findings and data collected. Monitor, manage illness and promote health. Providing education and counseling to patients and families in preventative care, medical conditions, and use of prescribed treatments and drugs. Assess, stabilize, and determine disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call service. Develop and order plans of care as appropriate. Provide counseling and education to patients, families and caregivers. Provide ongoing consultation and education to staff nurses, medical team and student trainees. Provide consultation support to Veterans referred to Cardiology Care. Identifies needs of the individual, family or community as a result of the evaluation of the collected data. Prescribe medications according to VA regulation and controlled substances within the guidelines of the Federal Controlled Substances Act. Initiates and adjusts therapeutic regimen as needed. Prescribe devices, supplies and durable medical equipment as appropriate. Order and interpret diagnostic testing and therapeutic modalities, including, but not limited to laboratory tests, EKG, and radiological studies. Drawing blood specimens for testing and performing other comparable lab procedures. Order VA specialty consults and ancillary services when appropriate. Conduct & document goals of care conversations and place corresponding life sustaining treatment (LST) progress notes and orders. Write State Authorized Portable Orders (SAPO) as allowed per state. General wound care. Work Schedule: Monday-Friday 8:00AM-4:30PM *Note-This position currently has a Return to Office (RTO) mid-term extension (180-days). Remote work is currently authorized; however, candidates may be subject to return to office in the event policy changes or the exemption is not extended.
