Pharmacist- Clinical Pharmacy Specialist

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties include but are not limited to: Implementation and monitoring of therapeutic drug plans. Obtain medication histories from patients and summarize significant findings in the medical record. Serve as a provider with prescribing privileges as outlined in the individual's Scope of Practice. Review Patient medical record and interview patients to detect drug-related problems. Provide patient education about medications and related topics when appropriate Assess patients for drug therapy needs. Serve as Pharmacist in charge in the absence of Pharmacy supervisor. Conduct drug utilization review, quality assurance activities, and utilization reviews as required. Document clinical interventions in CPRS in a timely and professional manner. Promote and monitor compliance for established drug therapy policy. Provide excellent customer service to Veterans, caregivers, and internal customers. Ensure the V-IMPACT Hub Pharmacy Program is in compliance with all regulations and accreditations Assist in the coordination of V-IMPACT Hub Clinical pharmacy service including training residents and telehealth pharmacist. Work Schedule: 7 days on/7 days off (Overnight); from 7:00pm to 7:00 am (Note: Work schedules are determined and approved by the supervisor. The incumbent may be required to work various shifts on a permanent or temporary basis based on facility/patient care needs).
