Please review the job details below and then click “apply.” We're looking for someone to join our team as a AFS Case Manager (PAT) who: The PAT case manager works with families referred by the Kern County Department of Human Services to increase child safety through improving family stability and well-being. The PAT case manager facilitates a 2-year home-visiting parent education curriculum, conducts developmental assessments, links families with community resources, and encourages them to proactively engage in preventing the need for further involvement with Child Protective Services. Essential Functions: • Responds to assigned referrals to engage families in case management • Conducts intakes for program enrollment, assessments, program releases and confidentiality agreements • Conducts in-home parent education visits and regular communication with PAT families • Engages PAT families in the program using a trauma-informed approach to encourage growth and success • Provides group connections for PAT families to encourage peer
